At-home screening kits are available to those who do not have access to a biometric screening event in their area. A test kit is mailed to your home upon request. You collect your own blood sample through a simple finger stick test and return your sample in a self-addressed, stamped envelope. Your results are automatically loaded within 15 business days of returning your sample.
PLEASE NOTE: The at-home test kit does not include results for blood pressure, height, weight and waist circumference.
To order a Home Test Kit, visit the Kansas Wellness Portal. Choose the Home Kit Option. You are not able to order a Home Test Kit if you already have an appointment for an onsite screening event.

No worries, you can complete the CCE Controlling Blood Pressure module, CCE Managing Your Blood Sugar Module, and CCE Preventing High Cholesterol module in your HealthQuest portal for 6 credits each, to make up for testing outside of the healthy incentive range.
You can find these modules by clicking “Earn Your Incentive Credits” on the left side of your HealthQuest portal dashboard, and viewing activities under “Healthy Range Incentives + CCE Reasonable Alternatives.”
